Indukanta ghritam – An ayurvedic formulation for acid peptic disorder and longevity: A review

  Abstract 


Introduction: Acid peptic disorders are common conditions reported in daily clinical practice in India. Epidemiological study of 30,216 patients inferred 7.8% prevalence of peptic ulcer in India, while prevalence of gastroesophageal reflux is reported to be around 28% in South West India. Repeated gastric ulcers, perforation, or hemorrhage can cause intestinal fibrosis or cancer which can lead to death. As the disease occurs mostly in Madhyama vayas (16-70 yrs of age) and if this age is affected, longevity also be affected. Based on the clinical features, such manifestations can be compared with Amlapitta and Parinama shula. These diseases are preventable and, if not addressed at the right time, may burden the scenario hampering the quality of life of the individuals. Objective: To screen the activities of individual ingredients of Indukanta ghritam and to propose possible mode of action in APDs and longevity. Data Source: Charaka Samhita, Sushruta Samhita, Ashtanga Hridaya, Sahasra yoga, Sharangadhara Samhita and online data bases. Review Methods: The classical text books were reviewed manually and review was also done by referring various scientific journals and other databases. Results: The formulation is a combination of several drugs, i.e., Dashamoola and Panchakola etc. They are referred to be useful in Amlapitta, Parinama shula etc. conditions and latest researches have established activities like gastroprotective, antioxidant, immuno-modulation etc. inferring the use of the formulation the therapeutics of APDs, as well as a rejuvenating drug. Conclusion: Indukanta ghritam can prevent as well as cure the disease and can improve the quality of life. The herbal formulation can minimize the disease burden and possibly can play an important role as a classical rejuvenating drug.

  Introduction Top


Maintenance of health and management of the disease are prime goals of Ayurveda.[1] To achieve both objectives, Acharya Charaka has mentioned two types of drugs, i.e., drugs those maintain healthy state and those which can treat diseases.[2] Health is defined by the World Health Organization as a state of complete physical, mental, and social well-being and not merely an absence of disease or infirmity.[3] Srotasas are the important systems of the body that carry or circulate the Doshas and Dhatus and their elements throughout the body. They are the microchannels not only responsible for dispensing the nutrients to different tissues but also excrete secondary metabolites. Annavaha srotas, one among them, has been frequently described in the literature of Ayurveda that is evidenced through the works of Charaka, Sushruta, Vagbhata, etc.Exhaustive descriptions on various diseases afflicting this system in a great length can be observed in these classics.[4] With drastic changes in diet, lifestyle, increased industrialization, and pollution, the population is affecting with various kinds of diseases basically affecting Agni, the prime source of life.[5] Stress also plays a pivotal role in manifesting peptic ulcer with Helicobacter pylori,[6] and incidences of such somatic manifestations are rapidly increasing in the current scenario. Few important psychosomatic diseases, for instance, are peptic disorders, ulcerative colitis, essential hypertension, ischemic heart disease, etc.[7] that are becoming challenges for the medical profession. Acid peptic disorders (APDs) are common conditions reported in daily clinical practice. An epidemiological survey of 30,216 patients (41.7 ± 12.7 years; 66% of males) reported a prevalence of 7.8% for peptic ulcer disease in India, while the prevalence of gastroesophageal reflux disease (GERD) from South-West India was reported to be around 28%.[8],[9] Based on the clinical features, such manifestations can be comparable with Amlapitta and Parinama shula that are primarily focused areas of the Ministry of Ayush, New Delhi.[10] These diseases are preventable and, if not addressed, may burden the scenario hampering the quality of life of individuals. Although conventional medical science has ample of drugs for APD, they are known for certain limitations. These conventional molecules are known for side effects such as headache, bowel upsets, and disorientation.[11] On the other hand, Ayurveda has a huge number of time-tested formulations, which can be used in such manifestations. Indukanta ghritam is one such compound herbal formulations mentioned in the scheduled books of the Drugs and Cosmetics Act, 1940 that is well familiar in treating conditions of the gastrointestinal tract including Shula (~abdominal pain).[12] It is a widely practicing formulation in the Indian scenario with proven clinical efficacy of the formulation in cases of peptic and duodenal ulcers.[13],[14],[15],[16]

An attempt has been made to compile its ayurvedic concept and important activities, which made the drug helpful in gaining longevity due to effectiveness in APD.


  Materials and Methods Top


The current review was done from Charaka Samhita, Sushruta Samhita, Astanga Hridaya, Sahasrayoga, and Sharangadhara Samhita, respectively. The contemporary review was done from modern textbooks and scientific journals. Each ingredient was evaluated for their ayurvedic property/action (Karma) related to the management of APD and longevity (Rasayana).


  Review Results Top


Indukanta ghritam is a combination of 17 herbs, Godugdha (cow milk), Goghrita (cow ghee) and Yavakshara; and the method of preparation is well described in API.[17] The Rasa (taste), Guna (properties), Veerya (potency), Vipaka (biotransform), and Doshaghnata (effect on Doshas) of each ingredient are mentioned in [Table 1].

The ingredients have been also reported having anti-inflammatory, antimicrobial, antioxidant, antihepatotoxic, antiulcerogenic activities [Table 2].

Ghrita has the properties such as Pittanilahara (alleviating vitiated Pitta and Vata), Rasa-shukra-ojasahitam (beneficial for rasa, shukra, and oja), Nirvapanam (cooling), Mridukaram (softening), and Swara varna prasadanam (improves voice and complexion). It is also mentioned that Ghrita has Sanskaranuvartana (clenching properties of other drugs).[100] Indukanta ghritam is acclaimed in classics for Udara roga (diseases related to the stomach).[12]

Ingredients of Indukanta ghritam are used as a Rasayana at many places in classics, and they also have been mentioned in various Rasayana yogas as ingredients. Haritakyadi yoga,[101] Brahmarasayana,[102] Dwitiya Brahmarasayana,[103] Chyavanaprasha,[104] Haritakyadi rasayana,[105] Amalakavleha,[106] Vidangavaleha,[107] Amalakavleha apara,[108] Amalakayasa Brahmarasayana,[109] Pippali rasayana,[110] Pippali Vardhamana rasayana,[111] Dwitiya Indrokta rasayana,[112] Amalaka ghrita,[113] Bhallataka kshira,[114] Bhallataka taila,[115] Amalaka rasayana,[116] Bhallataka kshaudra,[117] Virechana yoga,[118] Abhayamlakavaleha,[119] Triphala rasayana,[120] Nalnadi ghrita,[121] Brahmyadi rasayana,[122] Chitraka rasayana,[123] Shunthyadi rasayana,[124] Somaraji rasayana,[125] Narasingha ghrita,[126] Narasingha taila,[127] Murvadi rasayana,[128] Gokshuraka rasayana,[129] Pancharavinda ghrita,[130] Chatuskuvalya ghrita,[131] and Bhallataka rasayana,[132] are the classical examples of Rasayana where the ingredients were used.


  Discussion Top


The drugs currently used in the treatment of gastric ulcers are antacids, anticholinergics, proton-pump inhibitors, mucosa protective agent, and H2-receptor antagonists.[133] Despite progress in conventional chemistry and pharmacology in producing effective antiulcer drugs, the traditional formulations may provide a useful source of new compounds that are used for development as pharmaceutical entities or, alternatively, as simple adjuncts to existing therapies. Several herbs and spices have been used to treat gastrointestinal tract (GIT) disorders, including gastric ulcers.[134] Many such formulations are mentioned in Ayurveda textbooks that may be useful in treating pathologies such as APD or GERD. Considering multi-ingredients with diversified therapeutic activities, Indukanta ghritam may have the upper hand against the conventional drugs. It has shown healing property, improved platelet aggregation, inhibit gastric acid secretion, inhibit neo-angiogenesis etc.[135] It is also effective in various symptoms of Parinama shula (duodenal ulcer), i.e., pain abdomen, nausea, vomiting, burning sensation in the abdomen, acid eructation, flatulence, indigestion, constipation, and tenderness in the epigastrium.[136] It is also having an immunomodulatory effect which can be utilized in various diseases and their side effects like cancer.[137] Ghrita has a property of Sanskaranuvartana, which means that the abovementioned properties of all drugs will be carried by Ghrita. All such unique qualities including Rasayana and other pharmacological properties of the formulation make it a promising drug in the management of acid-peptic disorders. Ghrita has properties such as Ojovardhaka, Rasayana, and Vayahsthapana, which suggests that it is enhancing the longevity. Rasayana dravya keep help tissue functioning in optimal level and keep the body in young age and improve quality of life.[138] Oja is the optimum Dhatus Teja which is said to be Bala (strength) of the body,[139] which means that it is responsible for innate immunity of individuals also. Thus, it helps in preventing individuals from diseases.

The data [Table 1] infers that Tikta, Katu, Madhura, and Kashaya are the dominant Rasas in the formulation. Tikta rasa pacifies vitiated Pitta. It is helpful in eliminating Daha (burning sensation in general) by Pitta shoshana (absorbing excess of Pitta). It has Deepana and Pachana (helps in stabilizing gastric functions at different levels) and Krimighna (antimicrobial) properties, which helps in maintaining the physiological flora of GIT.

Thus, overall Tikta rasa predominant formulation clears excessive gastric secretions, absorbs such excess quantities, keeps away the microbes, and helps in maintaining the physiological functioning of gut microflora.[140]

Katu rasa also helps in maintaining digestive functions (Deepana) and pacifies inflammation (Swayathu). Katu rasa is also familiar with its Vranavasadana (wound healing) karma,[141] while Madhura rasa also alleviates Pitta, pacifies burning sensation, and helps in wound healing.[142]

Studies on the individual components have been evaluated for their efficacy as antioxidant, antimicrobial, anti-inflammatory, and analgesic activities. Possibly, a combination of all these activities will synergistically work in multiple directions to and become responsible for gastroprotective activity and its efficacy in APD and GERD.


  Conclusion Top


Considering the review, it can be concluded that Indukanta ghritam may be effective in various disorders such as APD and GERD and also has Rasayana properties due to its ingredients. Thus, it may be helpful in antiaging and longevity and also improves the quality of life by minimizing the clinical features of various disorders.
